Open Adjusting Environment — This office utilizes an open adjusting environment for ongoing patient care. Open adjusting involves several patients being seen in the same adjusting room at the same time. Patients are within sight of one another and some ongoing routine details of care may be discussed within earshot of other patients and staff. If you prefer not to be adjusted in an open adjusting environment, other arrangements will be made. This environment is not used for taking patient histories, performing examinations, or presenting reports of findings — procedures conducted in a private, confidential setting.
You have the right to:
• Request restrictions on certain uses and disclosures of your PHI, including restricting disclosures to health plans when you have paid out-of-pocket in full.
• Request confidential communications, for example requesting that we not call your home number.
• Inspect and obtain a copy of your health records.
• Request an amendment to your records if you believe they are incorrect or incomplete.
• Receive an accounting of disclosures of your PHI.
• Obtain a paper copy of this notice, even if you have agreed to receive it electronically.
